          Deceased Person’s Name

          Name of Funeral Home servicing the family

          If Available, Date of the Home Going Service

          Contact Person’s Name and Phone Number

          Contact Person’s Relationship to the Deceased

          Is the Deceased a Member of OWCC, Yes or No
